OFCCP Releases Updated Voluntary Self-Identification of Disability Form

Contractors may continue to use the existing form until August 4, 2020

The Office of Management and Budget (OMB) approved use of a revised self-identification form on May 5, 2020. OFCCP hopes that the streamlined and revised Voluntary Self-Identification of Disability Form (CC-305) will increase the response rate of applicants and employees who choose to voluntarily self-identify their disability status. 

Contractors and subcontractors are required to solicit the voluntary self-identification of disability status of applicants and employees and the pre-and-post offer stages.  Employers must also re-survey employees at least every five years.   The form is now on a single page instead of two and can be made available to applicants and employees in either electronic or paper form. 

Federal contractors and subcontractors have until August 4, 2020, to implement the new form into their applicant and employee systems and processes.
